Daniel Cole, NP

Dan is a board certified Family Nurse Practitioner. He is a graduate of Purdue University Northwest with his Master's in Nursing. He obtained his board certification through AANP in 2020. Since then he has practiced in emergency medicine as well as helping to run an observation unit for over four years. He has also worked in an urgent care setting for almost two years. Dan is also certified in dry needling. Prior to becoming a Nurse Practitioner Dan worked as a registered nurse in cardiac ICU, surgery, cardiac surgery, and cath lab for nine years. Dan established Maple Leaf Health and Wellness in 2025 with the intent to provide high quality affordable healthcare to patients in his community. Dan strives to provide evidence based medicine along with functional medicine tailored to each individual. Dan married his wife Joni Cole in 2012 and they have two children, Nya Grace and Kohen Daniel. In his free time he loves to travel, cook, go to the gym, cheer on Notre Dame football, and attend and coach his kids sporting events.
