Nicole Allen, DO

Nicole Allen is a board-certified family physician and Fellow of the American Academy of Family Physicians at Mountain Roots Direct Primary Care in Spruce Pine, North Carolina.

She graduated magna cum laude from North Carolina State University before earning her medical degree, also magna cum laude, from the Edward Via College of Osteopathic Medicine in Spartanburg, South Carolina, in 2016. Her family medicine residency at Spartanburg Regional earned her the Resident Teacher of the Year Award in recognition of her commitment to medical education and mentorship.

Dr. Allen came to Mitchell County in 2019 to practice full-spectrum rural family medicine and later completed a Rural Medicine Fellowship at MAHEC in Asheville. She has taught as a clinical preceptor for UNC and MAHEC and presented on rural healthcare topics such as maternal-child health in Appalachia. In 2026 she founded Mountain Roots Direct Primary Care to bring personalized, relationship-based care back to rural medicine.
