Mohammed Rezai

Dr. Mohammed Rezai is a double board-certified physician at Zai Health in Thousand Oaks, California. He is a diplomate of both the American Board of Family Medicine and the American Board of Preventive Medicine, with a specialization in addiction medicine.

Rezai grew up in Northridge, California, then studied at UCLA before heading north to Touro University in Vallejo for medical school. From there he completed a rural family medicine residency at the Shasta Community Health Center in Redding, affiliated with UC Davis. That path has earned him recognition from the Ventura County Medical Association and a commendation from the California State Legislature for his work during the COVID-19 pandemic.

Rezai also gives his time to the community, staying active with several domestic violence shelters across Ventura County.
